HomeBase is a structured transitional living program (STLP) providing transitional living assistance for young men age 16-20 (services available until age 21).

The average length of stay is 6-18 months in a structured group home setting.

Residents receive:

  • individual, family, and group counseling
  • education and employment assistance
  • budgeting and money management
  • life and social skills training
  • recreation

Through the program, young men have the opportunity to participate in community activities and to work together to support each other, encouraging them to care about themselves and others. The program also helps young men develop an independent living plan and set goals for a positive future. After graduating the program, young men receive support as they work toward the goal to obtain their own housing.

The HomeBase program is available to young men who are at imminent risk of losing their current placement or who are at risk of out of home placement, or adolescents lacking family resources and in need of transitional living services.

Referral Information 

Self-referrals are accepted. Referrals also accepted through the Division of Child Protection and Permanency, community providers, and clergy. Youth referred through DCP&P may enter the program between the ages of 18-20. Self-referred, or non-system youth, or youth who are homeless may enter the program at ages 16-20.
