HomeBase/GrandSlam HomeBase/GrandSlam's structured transitional living program and supported apartments provide transitional living assistance for homeless youth ages 16-20. The primary goal is the development of an independent living plan. Youth stay six to 18 months in a structured group home setting and/or up to one year in a supported apartment where they receive counseling, life and social skills training, and employment and education assistance. |
